The enquiry form
The form asks for your name, email, the country where the patient lives, an optional WhatsApp number, and your consent. We use these details only to reply to and handle your enquiry.
Please do not send diagnoses, medical records or passport details through the form. The form is processed by Formspree, our form provider; see Formspree's own privacy policy for how it handles data.
Medical information
If you later choose to send medical information, we use it to handle your enquiry: preparing what a hospital needs for a review, and arranging agreed next steps. We do not diagnose or make treatment decisions.
Before records go to any hospital or other provider, we tell you who will receive them and why, and we ask for your permission. If you contact us for another person, please confirm you have their permission or are authorised to act for them.
The website
The website is hosted by Cloudflare and uses Google Analytics and Google Fonts. These services receive technical information such as your IP address, browser type and the pages you view. Form entries are not sent to analytics.
Email and WhatsApp
Email and WhatsApp are separate services with their own privacy terms. Ask us about a suitable way to share documents before sending anything sensitive.
Changes to this notice
We will update this notice as we add details on the records-sharing channel, how long information is kept, cross-border transfer safeguards and cookie choices. The date at the top shows the current version. Questions: care@wellin.pro.
